Not every fat reduction concern requires a device or a surgery. Some fat deposits are small enough — localised enough, accessible enough — that a precisely placed injectable solution is the most proportionate and most practical approach. The small accumulation beneath the chin that produces a double chin. The modest flanks that remain despite an otherwise lean physique. The inner thigh fullness that has always been there, regardless of training. The knee fat pad that catches the eye in profile. These are concerns for which cryolipolysis is a useful option but where the geometry is awkward, where the deposit is too small for full applicator coverage, or where the patient prefers an injectable approach over a device-based one.

Mesolipolysis occupies this clinical space specifically. Phosphatidylcholine — a naturally occurring phospholipid that is a constituent of all cell membranes in the human body — at the concentrations used in mesolipolysis disrupts the fat cell membrane in the treated zone, causing fat cell lysis and releasing the triglyceride content of the fat cells into the interstitial space. The released fat is metabolised and cleared by the lymphatic system over the 4–6 weeks following each session. The mechanism is selective for fat cells at the concentrations and volumes injected — the overlying skin, the nerves, and the underlying muscle in the treatment area are not meaningfully affected.

The post-session swelling is a normal and expected response to the fat cell lysis and the inflammatory cascade it initiates — and it is worth preparing patients for its significance. The treated area typically becomes visibly swollen and firm within hours of the session, reaching peak swelling at 24–72 hours and resolving progressively over 2–4 weeks. This is not a complication. It is the tissue's inflammatory response to the fat cell lysis that the treatment has initiated — and its severity is broadly proportional to the number of fat cells being lysed, which is in turn proportional to the size of the deposit being treated.

What Mesolipolysis Actually Does

The phosphatidylcholine-deoxycholate solution is injected in small aliquots in a systematic grid pattern across the target fat deposit — the needle accessing the subcutaneous fat layer at each injection point and depositing a precisely measured volume of the lipolytic solution. The solution disperses through the fat tissue from each deposit point, reaching the fat cells in the immediate vicinity and initiating the membrane disruption process that causes fat cell lysis. Over the 4–6 weeks following each session, the contents of the lysed fat cells are processed and eliminated — producing the progressive, even fat reduction that patients notice as the post-session swelling resolves.

Patients should be counselled to expect the swelling timeline clearly before treatment — because the double chin that looks more prominent at 48 hours post-session before it improves dramatically at 3 weeks post-session is a normal part of the treatment course, not a sign that something has gone wrong. Full results are not assessable until 4–5 weeks following each session — and we communicate this clearly at every consultation before treatment begins.

Submental fat — the double chin — is the most common indication and the one where mesolipolysis produces the most consistently dramatic and appreciated results. The double chin is anatomically well-suited to mesolipolysis: the fat deposit is accessible, precisely localised, and surrounded by tissue that tolerates the injectable approach well. Most patients treating the submental area require 2–3 sessions spaced 4–6 weeks apart for the desired degree of reduction.

Flanks, bra fat, and inner thighs — smaller, localised deposits that are too targeted for effective cryolipolysis coverage or where the patient prefers an injectable approach — are treated with a more extensive injection grid that distributes the lipolytic solution evenly across the deposit. Sessions for larger body areas require more injections and produce proportionally more significant post-session swelling, which is managed with appropriate aftercare and the realistic expectation that full results will not be visible until 4–6 weeks after each session.

Results, What to Expect, and the Swelling Reality

Results from mesolipolysis become fully visible as the post-session inflammatory response resolves — typically 3–5 weeks following each session. Before this point, the treated area may appear temporarily larger or more prominent than before treatment; this is normal and resolves as the cleared fat cells are eliminated and the tissue normalises. Patients who assess the treatment at 2 weeks post-session will not be seeing the final result. Patients who assess at 5–6 weeks will be.

A standard programme involves 2–4 sessions per treatment area spaced 4–6 weeks apart. The fat cells permanently destroyed by each session do not regenerate — the reduction in the treated deposit is permanent provided weight remains stable. Significant weight gain will expand remaining fat cells in and around the treated area, partially reducing the improvement.
