The face produces wrinkles and lines through three distinct biological mechanisms — and each mechanism requires a different clinical approach to address it effectively. The first is muscular: repeated facial expressions fold the overlying skin with every contraction, and over years, the skin's diminishing elasticity means that the fold no longer fully resolves when the expression relaxes. Dynamic lines — the frown lines, the forehead lines, the crow's feet — are the visible consequence of this process. The second is structural: the descent and deflation of the facial fat compartments creates folds and creases that exist at rest, independent of expression — the nasolabial folds, the marionette lines, the perioral area that deepens as the supporting structure above it descends. The third is intrinsic: the skin's own dermis thins, loses collagen and elastin, and develops the surface quality of fine lines and crepiness that reflects its biological deterioration.

The treatment that is most effective for the first mechanism — dynamic lines — is botulinum toxin, which addresses the muscle driving the line at its neuromuscular origin. The treatment for the second — structural folds — is volumetric restoration with dermal filler, Endolift, or structural lifting to address the tissue changes producing the fold from above. The treatment for the third — intrinsic dermal deterioration — is skin quality restoration: Profhilo, PDRN, laser resurfacing, MNRF, and skin boosters, each addressing a different aspect of the biological decline responsible for surface fine lines and crepiness.

Dynamic Lines — Muscle-Driven Wrinkles

Dynamic lines — frown lines between the brows, forehead lines, crow's feet, bunny lines at the nose bridge, and chin dimpling — are caused by repetitive contraction of the underlying facial muscles folding the overlying skin. Botulinum toxin, placed precisely in the responsible muscles, relaxes the contraction and allows the overlying skin to rest without the repeated folding that creates and deepens these lines. The result — appropriate relaxation without frozen expression — is the most specific and most effective treatment available for this category of line, because it addresses the cause directly at the neuromuscular level rather than the visible effect at the surface.

For dynamic lines that have become partially static — persisting to some degree at rest, not only with expression — botulinum toxin remains the primary treatment, often combined with either dermal filler for structural support of the static component or with Profhilo and laser for the dermal quality improvement that reduces the depth of the static line.

Static Folds, Fine Lines & Programme

Static Folds, Surface Fine Lines, and Comprehensive Programme

Static folds — the nasolabial folds, marionette lines, and the loss of definition along the lower face — exist at rest and reflect the structural changes of facial ageing rather than muscle activity. The most effective treatment is the one that addresses the structural cause: restoring the midface volume whose descent has produced the nasolabial fold, lifting the tissue that has descended to create the marionette crease, or using Endolift or HIFU to address the structural laxity of the supporting tissue. Filler placed directly into the fold may supplement the treatment of these lines but is most effective when used to refine a result already improved by addressing the structural cause.

Surface fine lines — the fine lines that appear across the cheeks, the periorbital area, and the upper lip as skin quality declines — reflect the thinning of the dermis rather than muscle activity or structural descent. These lines respond to the treatments that address dermal quality directly: Profhilo for bio-remodelling and collagen stimulation throughout the dermis; PDRN for regenerative stimulation through A2A receptor activation; laser resurfacing for the most significant improvement through direct removal of the affected superficial layers and thermal collagen remodelling beneath; MNRF for non-ablative deep collagen stimulation; and skin boosters for deep dermal hydration that physically plumps the tissue beneath the lines.
