Tattoo ink is permanent by design. The pigment particles are deposited in the dermis by the tattooing process at a size too large for the immune system's macrophages to engulf and clear — which is why they remain in place indefinitely under normal conditions. The skin around them heals; the ink stays. What makes laser tattoo removal possible is the same principle that makes all laser treatment work: selective photothermolysis. A laser pulse at the correct wavelength is selectively absorbed by the ink particles — heating them rapidly to the point of thermal expansion and fracture, or in the case of picosecond pulses, shattering them through photoacoustic shockwave — producing ink fragments small enough for macrophages to finally engulf and transport to the lymphatic system for clearance.
The process is not instant, and it is not always complete — and both of these facts deserve to be stated clearly at the outset. The ink must be fragmented across multiple sessions, because the total ink volume in a professional tattoo far exceeds what can be cleared in a single laser exposure. The cleared ink fragments must then be transported and eliminated by the lymphatic system — a biological process that takes weeks between sessions and is the reason that sessions cannot be scheduled too closely together. And some inks — particularly light blues, turquoises, and yellows — absorb laser energy less efficiently than dark inks, and may require more sessions to achieve the same degree of clearance, or may resist complete elimination even after a full treatment course.

What Tattoo Removal Actually Does
When a laser pulse of the appropriate wavelength and duration strikes an ink particle, it deposits its energy into the particle faster than the particle can dissipate that energy as heat — causing the particle to expand rapidly and shatter. In Q-Switch nanosecond mode, this shattering is primarily photothermal: the rapid heating and thermal expansion fractures the particle into smaller fragments. In picosecond mode, the pulse is delivered so rapidly — one trillionth of a second — that the thermal expansion occurs before significant heat transfer to the surrounding tissue, producing a photoacoustic shockwave that shatters the particle mechanically into significantly smaller fragments with less thermal collateral effect. The smaller the fragments produced, the more efficiently they can be cleared by macrophages — which is why picosecond mode is generally more effective per session and safer for the surrounding tissue than nanosecond mode alone.

What the Treatment Involves
Treatment sessions begin with topical anaesthetic applied to the tattoo area 30–45 minutes before the laser is used. The session itself is brief in duration relative to the weeks of clearance it initiates — typically 15–30 minutes for a medium-sized tattoo, less for smaller ones. Each laser pulse produces a sensation described by most patients as a sharp snapping heat — rapid and repetitive across the treatment area. Immediately following treatment, the tattoo area develops a white frosting — a CO₂ vapour response caused by the rapid heating of tissue water — which resolves within 20–30 minutes. Mild redness, occasional pinpoint bleeding, and sometimes superficial blistering over the following 24–72 hours are normal healing responses and are managed with the wound care instructions provided after every session.
Sessions are spaced a minimum of 6–8 weeks apart to allow the lymphatic system to clear the fragmented ink from the previous session before the next laser exposure.
